    Denial Management Services

    What Are Denial Management Services?

Denial management services are the processes a billing team uses to find out why insurers deny claims, correct and appeal those denials, and prevent repeat denials. Done well, they turn rejected claims into payments and use the data to stop the next denial.

A rejection and a denial are not the same thing. A rejection is a claim the payer refuses before processing, usually because of a data error, and it can be fixed and resubmitted. A denial is a claim the payer processed and refused to pay, so it needs a correction or a formal appeal.

Denials are common. In the Experian Health 2025 State of Claims survey, 41 percent of providers said 10 percent or more of their claims are denied.

Common Denial Reasons and How We Fix Them

Most denials trace back to a short list of causes. Our denial management services sort every denial by reason code, so we fix the cause and not only the claim.
    Denial reasonCommon examplesHow we respond
Missing or inaccurate claim dataMissing information (CO-16), wrong modifier, missing provider IDCorrect the data, resubmit and add a scrubbing rule so it cannot repeat. See our CO 16 denial code guide.
Authorization problemsNo prior authorization or precertification (CO-197)Request a retro authorization or appeal with clinical notes.
Registration and eligibility errorsCoverage ended, wrong subscriber ID, duplicate claim (CO-18)Verify coverage, fix demographics and resubmit with proof.
Coding errorsBundled services (CO-97), diagnosis not linked to the procedureReview coding, correct the claim or appeal with documentation.
Medical necessityService not deemed medically necessary (CO-50)Appeal with clinical records and the payer policy. See our CO-50 guide
Timely filingClaim filed after the payer deadline (CO-29)Send proof of timely filing and prevent repeats with fast submission.

How Our Denial Management Services Work

Our denial management services follow a six step process, so every denied claim has an owner, a deadline and a next action.

    1. Capture and sort

We pull denials from ERAs and payer portals and sort them by reason, payer, deadline and dollar value.

    2. Root cause analysis

We find why the claim was denied: data, coding, authorization, eligibility or payer policy.

    3. Correct and resubmit

Fixable claims are corrected and resubmitted quickly, before the filing window closes.

    4. Appeal with evidence

We write appeals with clinical records, payer policy and contract language attached.

    5. Escalate and track

Stalled appeals go to payer supervisors and external review, with every deadline tracked.

    6. Prevent repeats

Findings feed back into scrubbing rules, front desk fixes and monthly denial reports.

Denial Prevention: Stop Denials Before They Happen

The cheapest denial is the one that never happens. Our denial management services include prevention steps that work before the claim is sent.

    • Eligibility and benefits checks before the visit
    • Prior authorization tracking
    • Claim scrubbing with payer specific rules
    • Coding review by certified coders
    • Clean claims submitted within 72 hours of service
    • Weekly review of denial trends by payer

Signs You Need Denial Management Help

These warning signs usually show up before the revenue loss becomes obvious.

    Denial Metrics We Track

You cannot fix what you do not measure. We report these numbers by payer every month.

    • Initial denial rate: denied claims divided by claims submitted.
    • First pass resolution rate: claims paid on the first submission without rework.
    • Appeal overturn rate: appealed denials that are reversed in your favor.
    • Denial write off rate: denied dollars that end up written off.
    • Days to resolve a denial: time from denial to payment.
    • Top denial reasons: the codes behind most of your lost revenue.

These numbers feed into your medical AR management and are included in our billing reports.

Out of Network Denials and IDR

Out of network claims need more than a standard appeal. Payers often reprice them or pay far below billed charges, and the reason is hidden in the EOP and its remark codes.

Our denial management services for out of network claims review the EOP, negotiate with the payer or pricing vendor, and file Independent Dispute Resolution under the No Surprises Act when negotiation fails. Denial Management Services FAQs

Denial management services cover finding the cause of each denied claim, correcting or appealing it and preventing repeats. ZanexMed works denials by payer, deadline and dollar value, then uses the findings to improve claim scrubbing and front office processes.
A rejection is a claim refused before processing, usually for a data error, and it can be fixed and resubmitted. A denial is a processed claim that the payer refuses to pay, and it needs a correction or a formal appeal.
Appeal deadlines vary by payer and plan, and they often fall between 60 and 180 days from the denial date. Timely filing limits also vary. We track the deadline for every claim and start with the shortest window.
Many practices aim to keep the first pass denial rate under 10 percent, and lower is better. The right target depends on specialty and payer mix. A rising trend matters more than a single month.
Most billing companies charge a percentage of collections, typically 3% to 8% depending on volume and services. ZanexMed offers transparent plans with no hidden fees. See our pricing page for details.
Often yes, if the claim is still inside the payer appeal and filing window. During onboarding we review your denial backlog, sort it by deadline and value and work the recoverable claims first.
Yes. We review out of network denials and underpayments, negotiate with the payer and file IDR when it applies. Use the free IDR Claim Checker to see if a claim qualifies.
Access to your practice management system or EHR, recent ERAs or denial reports, and your payer contracts if you have them. Onboarding usually takes 30 to 60 days.
